Zing Health offers Medicare beneficiaries three (3) easy ways to enroll in our plan.

Once you have made the decision to enroll in Zing Choice IL (HMO), select one of the following options:

Option 1 – Schedule an Appointment

Schedule an appointment with a “trusted health advisor” to review benefit options today

Contact Customer Service to schedule an appointment with a Zing Benefit Consultant

8 a.m. to 8 p.m. Monday through Friday and

7 days a week October 1st through March 31st

Option 2 – Download and Mail

Download our enrollment application.

Once you have downloaded our enrollment application, complete, sign and return by mail to:

Option 3 – CMS Online Enrollment Center

Medicare beneficiaries may enroll in our plan by visiting Centers for Medicare and Medicaid Services (CMS) Online Enrollment Center located at http://www.Medicare.gov.

Zing Choice (HMO) is a Medicare Advantage plan with a Medicare contract. Enrollment in Zing Choice IL (HMO) depends on contract renewal.
