Zing Choice
(MAPD HMO)
Available in Illinois, Indiana, and Michigan
The Zing Choice (HMO) plan is a coordinated care plan designed to lower the out-of-pocket cost of its members by utilizing network providers and limiting cost share for certain medical services. Members are required to select a primary care physician (PCP) who will coordinate care except for emergencies, out-of-area renal dialysis, and urgent care. Services received by out-of-network providers may not be covered. Supplemental benefits including but not limited to dental, vision, hearing, transportation, and wellness programs are covered in-network only.
Zing Open Access
(POS HMO)
Available in Illinois, Indiana, and Michigan
The Zing Open Access (HMO-POS) plan is a coordinated care plan (CCP) with the point-of-service (POS) option. Members of this plan must choose an in-network primary care provider (PCP). The POS option allows members to access out-of-network providers for all other Medicare-covered services as needed. Members will pay the same cost share as in-network for most services received out-of-network. This plan has a low monthly premium designed to meet the unique needs of Medicare beneficiaries who qualify for Part D Low-Income Subsidies (LIS). Supplemental benefits including but not limited to dental, vision, hearing, transportation, and wellness programs are covered in-network only.
Zing Essential Wellness Diabetes and Heart (CSNP HMO)
Available in Illinois, Indiana, and MichiganThe Zing Essential Wellness Chronic Special Needs Plan (HMO C-SNP) is designed to meet the unique needs of members with certain chronic conditions. To be eligible for this plan, the enrollee must have Diabetes Mellitus, cardiovascular disorders, and/or Chronic Heart Failure. Members of this plan are required to select an in-network primary care physician (PCP) who will coordinate care except for emergencies, out-of-area renal dialysis, and urgent care. Services received by out-of-network providers may not be covered. Nurse Care Managers will assist members with managing their chronic conditions, provide effective clinical case management and care coordination to improve the members’ quality of life. The plan includes a specialized Part D formulary designed to meet certain chronic conditions. In addition, the plan includes specialized supplemental benefits and weight management programs designed to serve the specific needs of its members and are covered in-network only.
Zing Signature Care
(POS HMO)
Available in Cook County, Illinois And Marion County, Indiana
The Zing Signature Care (HMO-POS) plan is designed to provide members with prevention-focused primary care aligned with driving better health outcomes. Members of this plan must select an in-network primary care provider (PCP). The POS option allows members to obtain care from out-of-network providers for certain Medicare-covered services as needed. Members may experience higher cost share for certain services obtained out-of-network. Additionally, this plan includes Part D prescription drug coverage, wellness programs, and higher supplemental benefit allowances for better care efficiencies.
Zing Dual Complete Plus
(DSNP HMO or POS)
HMO-POS available in Indiana and HMO available in Michigan
The Zing Complete Plus (HMO-DSNP) Dual Special Needs Plan is intended to meet the needs of members who receive Medicare and certain Medicaid benefits. To be eligible for this plan the enrollee must be eligible for Medicare and full Medicaid. This plan is designed to coordinate the Medicare and Medicaid benefits of its members. Generally, members will pay 0% cost share for Medicare-covered services when covered by both Medicare and Medicaid. Members of this plan are required to select an in-network primary care physician (PCP) who will coordinate care except for emergencies, out-of-area renal dialysis, and urgent care. Services received by out-of-network providers may not be covered. Members will have access to clinical nurses called Care Managers. Care Managers work in conjunction with members and their healthcare providers to close gaps in care and support individualized treatment plans geared toward optimizing health related outcomes. The plan includes Part D prescription drug coverage. Supplemental benefits and weight management programs and are covered in-network only.
The Zing Premium Give-Back
(HMO)
Available in Illinois, Indiana, and Michigan
The Zing Premium Give-Back (HMO) is a coordinated care plan designed to reduce the monthly Medicare Part B premium. This reduction is set up by Medicare and administered through the Social Security Administration (SSA). Depending on how the enrollee pays their Medicare Part B premium, the reduction may be credited to their Social Security check or credited on the Medicare Part B premium statement. Members of this plan are required to select an in-network primary care physician (PCP) who will coordinate care except for emergencies, out-of-area renal dialysis, and urgent care. Services received by out-of-network providers may not be covered. This plan includes Part D prescription drug coverage and limited supplemental benefits which are covered in-network only.
Zing Dual Platinum Plus
(DSNP POS)
Available in Marion County, Indiana
The Zing Dual Platinum Plus (HMO-POS DSNP) is a Dual Special Needs Plan intended to meet the needs of members who receive Medicare and certain Medicaid benefits. To be eligible, the enrollee must have Medicare and full Medicaid benefits. This plan is designed to provide members with access to a high-touch, prevention-focused primary care. Members are required to select an in-network primary care provider. The POS option allows members to obtain care from out-of-network providers for certain Medicare-covered services as needed. Generally, members will pay 0% cost-share for Medicare-covered services when covered by both Medicare and Medicaid. Members will have access to clinical nurses called Care Managers.
